From Fertility and Sterility

Advanced maternal age was associated with an annual decline in reproductive success despite use of donor oocytes: a retrospective study

This paper set out to study if there is an increasing risk of poor reproductive outcomes associated with age in in vitro fertilization (IVF) patients using donor oocytes, and when it becomes clinically relevant. Challenging traditional paradigms that embryo factors drive the detrimental effect of age on reproductive outcomes, this study reveals that uterine age may also play a significant role in age-related detrimental effects. Given that embryo factors were controlled for, these findings suggest that the worsening of reproductive outcomes is associated with the effects of age in the uterus. In addition, this study design identified the specific age at which the live birth rate decreases, and the implantation failure rate and pregnancy loss rate increase.

Outcomes of ovulation induction in obese women with infertility after bariatric surgery

This study compares pregnancy rate (PR) and clinical pregnancy rates between obese women with a history of bariatric surgery and nonoperated controls undergoing ovulation induction (OI). Women with surgical weight loss in the case group and the 2 nonoperated obese control groups had pretreatment BMIs of 34.67, 41.51, and 35.83 kg/m2, respectively. Each underwent OI cycles. No significant differences were found in the mean follicular phase lengths (11.13, 13.39, and 12.14 days) and peak estradiol levels (166.17, 278.36, and 246.43 pg/mL). The outcomes of cumulative pregnancy rate were 12.50%, 10.00%, and 20.59% across the 3 groups. The cumulative CPR showed a trend toward improvement in group 3 (9.38%, 6.25%, and 19.12%). Among women with infertility undergoing OIs, surgical weight loss may improve CPR despite a persistent obese postoperative BMI.

Fertilin peptide decreases embryo resorption and improves live birth rate in mouse

This study looks at the effect of the mouse Fertilin peptide, on mouse in vitro and in utero embryo development. In mice, Fertilin 100 μM accelerated blastocyst formation in vitro, and reduced embryo resorption in vivo from 48.6% to 33.0%. Consistently, the live birth rate was increased from 42% to 63%. This study demonstrates that Fertilin peptide accelerates embryo development in vitro, reduces the resorption rate in utero, and increases the live birth rate in mice. In the mouse model, Fertilin appears to be the first molecule able to significantly reduce miscarriage in vivo and improve live birth rate.

Final oocyte maturation for in vitro fertilization: a comprehensive review with investigators’ recommendations

The use of gonadotropin-releasing hormone (GnRH) antagonist protocols in assisted reproductive technology provides various options for achieving final follicular maturation. The choice of agent should be tailored to the patient characteristics and the mechanism of action of the triggers. The two primary agents used for this purpose are human chorionic gonadotropin, which acts as a luteinizing hormone analogue, and GnRH agonist, which induces the release of internal gonadotropin stores. Although these agents aim to achieve the same goal, they differ in the mechanism of action and downstream effects. Understanding the underlying mode of action of the triggers is crucial for tailoring individualized treatment protocols, achieving optimal oocyte yield and quality, enhancing clinical outcomes while appropriately mitigating the risks. The aim of this review is to provide an in-depth look at each ovulation trigger agent and their combination and summarize recommendations on the basis of the most common patient characteristics.
